Inserting new Main Menu popup

I've created and maintained a developers' add-in for VS2005 and VS2008 for years, and it still works well with VS2010.  However, I know that's a temporary accommodation, and that in the future I should rewrite it as a WPF-based add-in.

So I've been looking for SDK samples to get me started, and can't come up with anything that shows how to add a new item to the main menu.  I don't want to bury our add-in in the Tools menu, which seems to be a default, nor do I want to (only) put a new button in the main button bar.  (Although a main menu item and a toolbar button would be fine.)

Could someone please point me towards an example that will get me started with a WPF add-in that modifies the main menu?  Thanks!
